Primary care is a place to discuss new concerns, stay current with prevention, and manage ongoing conditions. Our team helps connect each visit with follow-up and specialty care when needed.
Discuss symptoms and health goals; review medical, family, and medication history; check vital signs; and complete an exam or order tests when clinically appropriate. Visits can also address common illnesses and changes in health.
Review health risks and goals, immunizations, nutrition, activity, sleep, and other health habits. A Medicare Annual Wellness Visit focuses on a personalized prevention plan and differs from a routine physical; we help patients understand the visit and coverage that apply to them.
Discuss screenings based on age, history, and individual risk, such as blood pressure, diabetes, cholesterol, certain cancers, and HIV or other infections. The team can arrange appropriate tests, explain results, and coordinate any needed follow-up or referral.
For conditions such as diabetes, high blood pressure, kidney disease, asthma, or heart disease, the team works with each patient on goals, follow-up visits, medication review, monitoring, test results, and changes to the care plan when needed.
When a concern needs additional expertise, we help identify the right specialist or service, share relevant information with the patient’s permission, support scheduling and follow-up, and bring recommendations back into one coordinated care plan.
